Rogers' diffusion of innovation theory is a particularly good theoretical framework to apply to an EBP project. However, students may also choose to use change models, such as Duck's change curve model or the transtheoretical model of behavioral change. Other conceptual models presented such as a utilization model (Stetler's model) and EBP models (the Iowa model and ARCC model) can also be used as a framework for applying your evidence‐based intervention in clinical practice.

Evidence-Based Practice Proposal - Section F: Implementation Plan

 

In 500‐750 words (not including the title page and reference page), provide a description of the methods to be used to implement the proposed solution. Include the following:

1.      Describe the setting and access to potential subjects. If there is a need for a consent or approval form, then one must be created. Although you will not be submitting the consent or approval form(s) in Topic 5 with the narrative, the consent or approval form(s) should be placed in the appendices for the final paper.

2.      Describe the amount of time needed to complete this project. Create a timeline. Make sure the timeline is general enough that it can be implemented at any date. Although you will not be submitting the timeline in Topic 5 with the narrative, the timeline should be placed in the appendices for the final paper.

3.      Describe the resources (human, fiscal, and other) or changes needed in the implementation of the solution. Consider the clinical tools or process changes that would need to take place. Provide a resource list. Although you will not be submitting the resource list in Topic 5 with the narrative, the resource list should be placed in the appendices for the final paper.

4.      Describe the methods and instruments, such as a questionnaire, scale, or test to be used for monitoring the implementation of the proposed solution. Develop the instruments. Although you will not be submitting the individual instruments in Topic 5 with the narrative, the instruments should be placed in the appendices for the final paper.

5.      Explain the process for delivering the (intervention) solution and indicate if any training will be needed.

6.      Provide an outline of the data collection plan. Describe how data management will be maintained and by whom. Furthermore, provide an explanation of how the data analysis and interpretation process will be conducted. Develop the data collection tools that will be needed. Although you will not be submitting the data collection tools in Topic 5 with the narrative, the data collection tools should be placed in the appendices for the final paper.

7.      Describe the strategies to deal with the management of any barriers, facilitators, and challenges.

8.      Establish the feasibility of the implementation plan. Address the costs for personnel, consumable supplies, equipment (if not provided by the institute), computer related costs (librarian consultation, database access, etc.), and other costs (travel, presentation development). Make sure to provide a brief rationale for each. Develop a budget plan. Although you will not be submitting the budget plan in Topic 5 with the narrative, the budget plan should be placed in the appendices for the final paper.

9.      Describe the plans to maintain, extend, revise, and discontinue a proposed solution after implementation.

The transition to any evidence-based practice requires a proper strategic approach in persuading the health care service providers to adopt the proposed intervention; however, the success of the transition is dependent on the willingness of the health care provider to adopt the new intervention in its early stages (Mohammadi, Poursaberi, and Salashoor, 2018). In this study, the transition of hospitals in using the WHO checklist to prevent wrong-site surgery needs to have a good theoretical framework of implementation, as the willingness of these health care services to use the WHO checklist depends on the results of the checklist’s early adoption. In this intervention, one of the most favorable conceptual frameworks that will be used is Rogers’ diffusion of innovation theory. The diffusion of innovation theory will serve as the framework to identify the conditions in which health care services will support the early adoption of newly introduced practices and, also, to know which of the conditions requires additional effort for adoption to occur (Mohammadi, Poursaberi, and Salashoor, 2018; Sanson-Fisher, 2004). This theory is a special system of communication by which the newly introduced process or innovation is communicated through subsequent channels among the different members of a channel’s social group over time (Rogers, 1983). In other words, the people in a particular social group share their acquired knowledge and experience with one another in order to reach a mutual understanding accepting or rejecting the proposed intervention.
According to the Rogers’ diffusion of innovation model of the innovation-decision process (see Figure 1), there are five sequential stages on which an individual’s decision is affected. First, knowledge is received when the individual or organization is introduced to the existence of the newly proposed practice and gains significant understanding on how the practice works. Next, persuasion is developed when the individual or organization perceives the proposed practice as favorable or unfavorable. Then, a decision is made on whether the practice will be adopted or not. Subsequently, if the decision is in favor towards the adoption of the new practice, it will be implemented, where the new practice is put into use; and, finally, confirmed, where the new practice will be reinforced. In this study, the knowledge stage includes the awareness that harm to patients is done by wrong-site surgery each year; therefore, the WHO developed a universal checklist to avoid the occurrence of these events (Collins et al., 2014). Next, the persuasion stage occur since there is evidence-based research, such as the findings of Haynes et al. (2009), Weiser et al. (2010), and van Klei et al. (2011), where there is significant reduction in the occurrence of wrong-site surgery due to the use of the WHO checklist. The evidence will serve as a strong basis in the decision-making stage for the WHO checklist to be implemented and confirmed to avoid wrong site surgery occurrences.
